How to use Fields blocks in Menti Form

Collect contact details, addresses and other related information in one question in your Menti Form.

A Fields block groups several labeled input fields under one question, so respondents type a separate answer into each field, and their responses are grouped together per participant.

You can use a Fields question to collect contact details, addresses or registration information in a clear, consistent structure.

How to create a Fields block in Menti Form

To create a Fields question, click the + Create button in the top-left corner of the editor and select Fields under Questions.

Edit your Fields block

Click directly in the question field on the canvas and type. You can also add a description in the Add a description field just below the question. Use the description to explain why you are collecting the information or how you will use it.

To edit a field, click the label and add the question or the information you want to collect. To add another field, click + Add field below the existing fields.

When to use a Fields question

A Fields question works best when you need several short, related answers from each respondent. Because each answer is saved in its own field, the results are easier to read and compare than one long Open Ended answer.

A Fields question is useful for:

  • collecting contact details, such as name, email address and phone number

  • collecting postal addresses in a consistent format

  • gathering registration or sign-up information for an event

  • asking for several related details without creating a separate question for each one

Examples:

  • “Address” with the fields Street, City and Postcode

  • “Contact details” with the fields Name, Email and Phone number

  • “About your organization” with the fields Company name, Department and Job title

To collect one longer written answer, such as feedback or ideas, use an Open Ended question instead.

Additional functionalities

In the Edit tab on the right side panel, you can adjust more settings for your Fields block. You can make responses required, so respondents cannot move on without responding. You can also add an image to the Fields block.

View the results of your Fields question

Fields question results are shown in the Responses view, with one card per respondent. If a respondent leaves a field empty, the card shows No response for that field.
