Menti Form is a great choice when you want to build a survey-style experience that people can complete in their own time. Unlike a Menti Live presentation, where the presenter guides participants through slides in real time, a Menti Form is designed for asynchronous input and structured data collection.
Create a new Menti Form
To get started, click the New Menti + button on your dashboard and select Menti Form.
A new form will open in the editor with your first question ready to edit.
From here, you can add questions, text blocks, pages, and images to build out your form.
Add and edit questions and content blocks
To add a new question or content block, click on the + Create button in the top-left of the editor.
You can choose three question types: Multiple Choice, Open Ended and Scales (coming soon). You can also add a Text or Image block, or add a new Page to the form.
Once added, click into the question or text field and type your question or text.
You can also add a short description underneath to give respondents extra context or instructions. For example, you might use the description field to explain what kind of answer you are looking for, how the answer will be used, or whether the question is optional.
For Multiple Choice questions, click into the answer option fields and add your answer options there. You can also add more answer options by clicking the + Add option button at the bottom.
Choose your content type
Menti Form supports different types of content depending on what kind of information you want to collect.
Multiple Choice questions
Use Multiple Choice when you want respondents to choose from a set of predefined options.
This is useful for:
collecting clear, structured answers
segmenting respondents by team, role, location or experience level
asking preference questions
creating quick pulse checks
making results easier to compare and analyze
Examples:
“Which team are you in?”
“How long have you worked here?”
“How did you hear about us?”

Open Ended questions
Use Open Ended when you want respondents to write their own answer.
This is useful for:
collecting detailed feedback
asking follow-up questions
gathering ideas, suggestions or concerns
understanding the reasoning behind a previous answer
giving respondents space to answer in their own words
Examples:
“What is your current role?”
“What would make this process easier?”
“Do you have any additional feedback?”
Open-ended questions are best when you want qualitative input rather than fixed answer choices.
Text blocks
Use Text blocks when you want to add information without asking a question or you want to add context to one of your questions.
This is useful for:
adding a welcome message
explaining the purpose of the form
giving instructions before a section
adding context between questions
thanking respondents or setting expectations
For example, you might start your form with:
"Welcome to this survey. This is to get to know the team members a little better."
Images
There are two ways to add images to your Form.
Create an Image block from the + Create button:

Pages
You can split your Form up unto different pages. Each page can have a collection of questions, images and text blocks, so it's up to you to decide what content you want to be together on each page. Participants can move back and forth between pages when responding.
This is useful when your form has several topics or when you want to make a longer form feel easier to complete. For example, you could create one page for background questions, one page for feedback, and one page for final comments.
Pages help keep the form organized and make it easier to manage the structure from the left-hand sidebar. You can drag questions and blocks around in the left hand panel and move them from one Page to another.
Menti Form vs. Menti Live
Mentimeter lets you create both Forms and Live presentations, but they are designed for different situations.
Use Menti Form when you want to:
collect responses asynchronously
create a survey-style experience
let respondents complete questions in their own time
gather structured feedback before or after a meeting, workshop, class or event
organize questions into pages and sections
Use Menti Live when you want to:
present to an audience in real time
control the pace of the experience as the presenter
run live polls, quizzes, word clouds, or interactive slides
show results during a meeting, class, workshop or presentation
In short, Menti Live is for live interaction in real time. Menti Form is for structured response collection in the respondents' own time.
